  Definition: flat urothelial hyperplasia is caracterized by a thickening of the urothelium which still shows orderly maturation to the surface and lacks cytologic atypia.
  Flat urothelial hyperplasia may occur de novo, where it appears to have no malignant potential.
  It may also be seen in conjuction with cystitis , bladder calculi , and urothelial tumors.
  Several fragments/foci should be examined to prevent overdiagnosing a single focus with tangential sectioning as flat (...)

  Definition: High-grade papillary urothelial carcinoma is a neoplasm with urothelium lining papillary fronds, a predominant disorderly pattern and moderate to marked architectural and cytologic atypia.

  Low-grade papillary urothelial carcinoma shows minimal cytologic and architectural atypia. Adjacent papillary fronds may fuse.
  In a low-grade papillary urothelial carcinoma, the presence of scattered hyperchromatic nuclei and typical mitotic figures distinguish this lesion from the PUNLMP.
