May be due to ischemia with secondary granulomatous reaction and scarring
In a histologic review of adult epididymides obtained at autopsy (both epididymides of 408 men) or during surgery (261 men with testicular or epididymal nontumoral pathology), a peculiar granulomatous lesion was observed in two autopsy specimens (unilateral) and three surgical specimens. hemorrhagic fluid-filled cyst
Microscopically, stromal-filled papillae were lined by low cuboidal to columnar epithelium.
Epithelial cells are reactive for cytokeratin 7, cytokeratins AE1/3, and focally in the apical cytoplasm for CD10.
Focal CD10 reactivity is also noted in the stroma.
The lesion is negative for alpha-fetoprotein.
Differential diagnosis
metastatic renal cell carcinoma.
