Definition: This lesion is the microscopically the same as "metaphyseal fibrous defect" (cortical defect) and "non-ossifying fibroma" of bone. Their clinical and radiological features are rather different, however.
Synopsis
storiform pattern of spindled cells
frequent foam cells
foam cells with delicate fibrous stroma and reactive bone
variable benign multinucleated giant cells
- resembles cutaneous counterpart

Differential diagnosis
non-ossifying fibroma / metphyseal fibrous defect / fibrous cortical defect
atypical benign fibrous histiocytoma (Unni)
malignant fibrous histiocytoma of bone (MFH of bone)
desmoplastic fibroma
low-grade fibrosarcoma
late stage of eosinophilic granuloma (Langerhans cell histiocytosis )
brown tumor of hyperparathyroidism
giant cell tumor of bone
osseous xanthoma
non-ossifying fibroma / metphyseal fibrous defect
- similar histology but characteristic radiologic findings,
- usually younger than age 20 years,
- metaphysis of long bones
