Differential diagnosis

- low-grade endometrial stromal sarcoma ( LGESS )

  • Low grade endometrial stromal sarcoma is invasive of the surrounding tissues and vessels.

- endometrial stromal nodule

  • endometrial stromal nodule has large, thick-walled blood vessels with muscular walls.
  • Endometrial stromal nodule is non-invasive.
  • IHC is positive by IHC for CD10, ER, PR, androgen receptor, B-catenin, WT1, CK, vimentin, and muscle markers including muscle-specific actin and SMA.

- uterine cellular leiomyoma

  • Cellular leiomyoma has large, thick walled blood vessels with muscular walls, lacks atypia and mitoses.

- uterine leiomyosarcoma

  • Leiomyosarcoma is positive by IHC for desmin, SMA, myosin, ER, PR, EMA and Ki-67, and is invasive.
