Definition : Primary adrenal leiomyosarcoma is a exceedingly rare adrenal tumor thought to be originated from smooth muscle wall of central adrenal vein. SMA and desmin IHC is diffusely positive.

Differential diagnosis

- sarcomatoid adrenal cortical carcinoma
- malignant PEComa
- dedifferentiated liposarcoma
- adrenal metastasis
