Acantholytic actinic keratosis may mimic other conditions associated with acantholysis, such as Grover disease, Darier disease, and warty dyskeratoma, as well as seborrheic keratosis with acantholysis.

In the acantholytic actinic keratosis, there are clefts, usually suprabasal in location, although the change may be more widespread with acantholytic and dyskeratotic cells resulting from anaplastic/atypical changes producing disrupted intercellular bridges.
