Perinatal hypoxic/ischemic spinal cord injury

Synopsis

- acute hypoxic/ischemic spinal cord injury
- asphyxiated neonates

- spinal cord infarction

  • associated with prematurity
  • associated with episodes of systemic hypotension
  • lumbosacral cord segments more severely affected
  • affected levels central cord parenchyma completely necrotic with relative sparing of the periphery

- diffuse neuronal necrosis

  • ventromedial neurons most profoundly injured
  • hematomyelia dissecting into spinal cord parenchyma
  • germinal matrix hemorrhage
  • "Watershed zones" in the cord
